Need to re-order your pet’s medication?

Please use this form to request a repeat prescription for your pet’s medication. As by law, all prescription medications must be prescribed by a Veterinarian, and these laws also dictate certain conditions that must be followed. Not all requests may be able to be filled if these conditions are not met. For instance, if an animal has not been seen in our clinic for more than 6 months or medication is required for a unseen condition, your request has to be legally declined and an in person, with the animal, visit is required.
